UnitedHealth Group is the largest health-care company in the world by revenue, and it sits on both sides of the American medical system. One half, UnitedHealthcare, is the country's biggest health insurer, covering tens of millions of people. The other, Optum, delivers the care and the plumbing: it runs clinics, employs physicians, manages pharmacy benefits and sells data and technology services to the rest of the industry. Owning both the insurer and the provider gives the company enormous reach and has drawn political and regulatory scrutiny over its size and influence. Founded in 1974 and based in Eden Prairie, Minnesota, UnitedHealth is a fixture of the Dow and a bellwether for health-care policy. Its results hinge on medical costs, government reimbursement rates from Medicare and Medicaid, and the balance between the premiums it collects and the claims it pays.
